Below is the portal to Rank One the online medical forms system Fulton County Schools uses to manage sports physicals, insurance, and forms required for athletic participation.  Please take a moment to set up your account, register your athlete(s), and complete your forms.

Click "GHSA Sports Physical" for the current 2025-2026 school year sports physical form.  This form MUST be used per GHSA Bylaws and Fulton County Schools Athletics.
