Dragonfly Registration requires a Sports Physical. Download this form and get a physical for your athlete. * ALL pages of the physical must be uploaded into Dragonfly site
Hooch Booster Club Membership Form
On the form be sure and designate soccer as the sport to receive the club allocation donation.
